Registration
Registration for the 2009-2010 School Year (September 2009-June 2010) opens to the public on Monday, May 4, 2009.
To register your child(ren) into our program, please print off the Registration Form only, and mail the completed form to the address below with a check dated for July 1, 2009 for your registration and deposits (your registration and deposit checks can be combined onto one check). Your Casino Deposit check must be a separate check. Your checks should be payable to SSMDO (Southside Mother’s Day Out) and made out for the following amount:
- $30 non-refundable registration fee (one per family)
- $88 deposit per child
- So if you are registering:
- One child, your check should be for $118.00
- Two children $206.00
- Three children $294.00
- $250 deposit check for your Casino Shift dated for October 1, 2009
Deposit is returned to you at the year-end invoice if your child remains in the program for the full year, or is returned to you when you withdraw from the program, giving 4 weeks of written notice.
Southside Mothers Day Out has been granted their first Casino for the last quarter of 2009. Dates will be confirmed by the beginning of June 2009. ALL families will be required to work 1 shift at the Casino to help raise funds to enrich our program for our children. Your $250 Casino check will be returned when you Casino shift requirement has been fulfilled.
To speak to one of the registrars, please call 414-6899 and leave your name and phone number, and your call will be returned. It may take the Registrar up to 1 week to return messages left for her. Please do not leave additional messages as she will return your call as soon as she is able.
Once all of the registrations are sorted, you will be receiving a courtesy call by the middle of June to inform you of the spot your child(ren) has, or to let you know that they have been placed on our wait list. Once your child(ren) has a spot, you will receive a confirmation package (early to mid-July) with all of the information for your child(ren)’s first day and year.

Please note that space is limited in our program. Registration is taken on a first come first served basis. In order for your application to be considered complete and for your spot to be held, the registrar must have your registration form and your check.
